I did a replay the other week and noticed that the issue specifically tends to stem from a sidekick being "stuck" in lava or something else where they are stuck in a trigger_hurt situation. When that is going on the teleport exit fails. The teleporting code needs to be redone to allow teleporting regardless of their current goal.

Will look into it at some point, but not any time immediately in the near future as I'm in the middle of switching jobs and this is semi-undocumented feature.
