Docs update

Noticed that the help description for the patch-project command was the same as the patch-status command (copy paste error).

Also been using the commands a bit, so I thought I'd at least start the docs in the README file where it said TODO for you. I think I got it right, read through the code and it look straightforward enough.

Hum, BitBucket wouldn't let me push a branch, so here is a patch file instead.

Hrm, like Github you should be able to fork the repo, commit the branch to your fork, then submit a pull request back.
