Completion marks

Hello, In "course_output.php" you have two errors, so far:

line 821: must be "completion-y" instead of "complete-y",

line 824: must be "completion-fail" instead of "completion-n".

If a quiz is failed, the green check symbol should not be visible. Is confusing for users.

Red cross for "Fail" now appears if "Require grade" is set as a completion criterion., but "Require passing grade" is not set, and student fails. See explanation at https://moodle.org/mod/forum/discuss.php?d=344306#p1389679
