"Add to quick-access toolbar" context menu option does not work as expected

In the main menu, there is an option named "Add to quick access toolbar". It's a nice option which (since the users cannot modify the main RC ribbon) can give a user the ability to have some buttons added to the main window for quick access. This however does not work properly. It adds the button to the quick access toolbar but it seems an extra icon is missing because each button is added as a blank square.

Also, some of the main menu items (like "New database" and "update") cannot be placed to the quick access menu because no context menu is displayed for them on rightclick.
