move cactuscode.org content to einsteintoolkit.org

http://CactusCode.org contains lots of outdated information that rarely is updated due to lack of time. The proposal is to move the still relevant material to a sub-pages of einsteintoolkit.org

• http://einsteintoolkit.org/cactuscode.html is formatted too wide on large screens (past the width of the header bar), see attached screenshot
• http://einsteintoolkit.org/cactuscode-viz.html does not correctly mark hdf5_extract the way hdf5_merge is marked
• would be good if ExternalLibraries/HDF5 was made into a link
• the VisIt patch on http://einsteintoolkit.org/visualization/VisIt/index.html in the “Downloading and Installing” section is mis-formatted
