move tov example parfile out of cell in notebook into file in einsteinexamples

Currently the TOV example is a text embedded in the jupyter notebook https://github.com/nds-org/jupyter-et/blob/master/tutorial-server/notebooks/CactusTutorial.ipynb

This makes it hard to, unless one uses an “unnatural” ordering of operations to use MakeThornList tool to produce a minimal thornlist to compile to run the Hello, World! and TOV examples.
