What steps will reproduce the problem (cheats)? 1. Use .md ROMs from the No-Intro set 2. Use the .pat files provided by Mightymo from the downloads section 3. Rename the ROMs and/or .pat files to match 4. Put the ROMs anywhere you want (/genplus/roms by default) and the cheats in the /genplus/saves directory (or /cheats directory, doesn't matter either way). 4. Try to load the game and see if the .pat file for the game loads.
What is the expected output? What do you see instead? Expected? Cheats loaded from the .pat file. What do I see? Empty list.

It'll work fine when renaming the ROMs to .bin. But, by default it won't work with .md games.
