There were some leftover unresolved issues from pull request #155 that @jwise77 brought up. This gets rid of the 1000 lines of unused regions, and fixes up some docs. A lot of extra changed lines because I took @jwise77 suggestion of dim to a more meaningful variable name.

Is it worth updating enzo-stable with these changes?
Yes, I think so. I believe that obvious bug fixes like this should be routinely backported to enzo-stable.
This just cropped up again in a run I'm doing with enzo-stable. Did Greg's suggestion of backporting this ever happen?
I don't think so -- anyway I didn't do it. Sorry! Great idea, but I am time-constrained at the moment.
Based on a conversation going in IRC, I'm gonna hit this again. This needs to go into enzo-stable.
