Define hierarchical outline format

Use nested HTML ordered lists, plus something like an HTML data-attribute to store an associated scripture reference.

Decide on a standard scripture reference format - consider what Logos, BibleWorks, Bible Gateway's JavaScript tagger, etc., use.

Put one whole outline in one CouchDB document.

Write CouchDB views to present all outlines: * containing one verse, * contained in one chapter, and * contained in one book

Allow users to edit hierarchical outlines using a WYSIWYG editor like TinyMCE.
