[x264] Support x264 build 89 i_nal_hrd and b_tff options.

[x264] Update x264 support to build 89, and require build 89 or later. x264 build 89 now includes the NAL HRD patch, so the check for that patch has been removed from the configure script. The obsolete options [no]direct_8x8, [no]bidir_me, and [no]brdo are no longer recognized, and transcode will abort with an error if these options are used.

[tcexport][encode] Allow multiple frames to be returned from flush(). x264 (at least) can buffer dozens of frames, so change the encoder flush() interface to indicate whether a frame was returned, and loop over the flush() calls until all data has been output. libtcexport encapsulates this, so there shouldn't be any effect on core code.
