Chapter Preview Images Missing

I purchased the movie "Batman Forever" from the iTunes Store. It happens to be one of the few I have bought that does not have chapter markers. I was able to create a chapter track using Subler, and I put in the correct chapter names and times. I made sure that the "Create Preview Images" box was checked. After the chapter track was created, I went to file and saved it. I then tested my work in iTunes. To my surprise, the chapter preview images were missing. Instead of those beautiful thumbnail pictures by the chapters, there were empty grey boxes. I have spent a lot of time trying to figure out how to get the preview images to work with this movie. I have successfully created chapter preview images in other movies, which were actually mp4 files. The movies on the iTunes Store are m4v files. Do you know how to fix this issue so that the preview images will appear?

The movie from iTunes Store contains drm. So Subler can't decode the video, the only apps that can are iTunes and QuickTime Player.
