Applied index was not created automatically on Azure MongoDB

Great work on this package!

I’m using Azure Cosmos DB and version 2.2.0 of the library. Not used it before. When I run it on Azure, I get an error that an index on applied is missing. According to the documentation I should only have to create such index when I used the library before.

After I create the index manually, it works.
