Report viewing errors to the user instead of spinning indefinitely

If you attempt to view the attached DICOM instance with the OHIF viewer plugin, the thumbnail preview just spins indefinitely until Chrome tells you the page isn't responding. Instead, if the viewer can't display an image, it should just say so instead of locking up the browser.

    Thanks Charlie, the issue has been fixed in v3.1.0-RC-2. Apparently, there was a bug in the thumbnail image loader.
