Launchbox downloading media even when told not to do so

When I set launchbox to download Media, if I select the second option (only fetch the ones that are missing and KEEP what I already have), it is downloading the files again.

The same file, size, image, etc, just renaming the last one XXXX-1, XXXX-2, XXXX-345...

I left my onedrive syncing while it was downloading at home and then all my space was filled. I digged in the logs of onedrive and saw picures in my snes folders that were suppose to be there already and had this nasty surprise.
