Reduce reminder noise

I've had some complaints about spurious reminders since installing this plugin. Sometimes one reviewer is holding things up, but all reviewers (including the attentive ones) are receiving reminders. Or sometimes a pull-request author is slow to respond to comments.

Suggest adding an option to only send pull-request reminders if a reviewer has not made any comments since the most-recent commit.
