Returns 'broken' image for users without profile picture

After upgrading to Confluence 6.7.0 for users that have not uploaded a profile picture, the avatar server will return an empty response which to other applications shows up as a broken image.

Previously the avatar would show the standard grey placeholder image.

This appears to be due to a change of behavior in Confluence where the placeholder image is now an SVG injected by the UI itself and not an image returned by the profile picture API for the user.

It may be simplest to embed a placeholder image into the avatar server itself that can be returned when a profile picture is not found in Confluence itself.
