Fix CSS for comment entries

This is probably due to markup issues - IOW, the wrong markup is being used and therefore the appearance is wrong. For example:

NOTE: Username/ email isn't being shown properly. I'll fix that myself

Missing username/ email from the H2 is being caused by that not being inserted when the comment is created. Will log as a different ticket.

Fixed the missing username issue and gave these an appropriate heading level.

I think I've found Mike's intended styles, but something has bumped down the right column. Likely the width of something on the left.
