Update Calc(DB)ObservedMutations man pages

Jason Vander Heiden created an issue

1) binByRegions argument is redundant given is.null(regionDefinition), so it should be removed 2) with regionDefinition=NULL, full sequence is used for counting mutations, which is not made evident by man pages for calcDbObservedMutations or calcObservedMutations

    Could allow for regionDefinition=NULL argument in relevant functions that would then use the entire sequence as one region.

    Is this working now with regionDefinition=NULL? If so, the man page needs updating.

    binByRegions argument is removed and man pages updated to explain regionDefinition can be NULL for entire sequence to be considered.
