Checked again and quest works fine, I'm unable to reproduce these bugs.

He means player should use book, shield, sword and cain 4 items to active the 4 device, but they can just use for example only sword to active one device 4 times to pass the quest. and last NPC Solina's Evil Thoughts(id:32793)'s spawn location is a little bit far that player can not approaching to talk to her if they have geodata enabled.
