Sometimes doesn't add reference URL

I noticed that when I "confirm" an "Automatically matched" item, the claim is added without a reference, but if I create a "new item" (when a Q doesn't already exist), the claim is added with a reference.

When I associate a mix’n’match entry with a Q item via the up arrow (“[↑]”) on the entry’s page, it also adds without a reference.

Expected behavior is to always add a reference.
