image repetition in Related Media

I noticed that the Related Media contains lots of duplicates. For example:

http://tools.wmflabs.org/reasonator/?&q=362

Notice the repetition of:

http://commons.wikimedia.org/wiki/File:Flag_of_the_German_Empire.svg

[Sjoerd de Bruin]

Is this still happening? Can't reproduce on the provided item.
