Blows up completely if a file is removed in a patch

repo owner created an issue

For each hunk, after the patch has been applied to the temporary copy patchqueue tries to access the file in order to extract metadata (pygments-guessed langage) and then to include the file itself as a code block.

If the patch removed the file, these will obviously not quite work. For the second situation (LiteralInclude) Sphinx will warn that the file does not exist (or does it error?), but for the first it'll blow up completely with an IOError as it attempts to open a file which does not exist.
