Addon button not available in Firefox 22.0

After installing the addon, there is no button in the Add-on bar and it is not available to add from the Customize Toolbar dialog.

I just tried with a clean profile and it worked fine (Firefox 22 on Win 7). The icon showed in the lower right on the add-on bar and displayed an error since the URL was not set correctly.

I was the submitter of this issue. My problem was that I had Firefox configured to always use private browsing. Once I disabled that, the addon worked fine.
