Add user defined words to dictionary

Possible workflow: * it goes to a moderator, which will add the words later (can be several hours / 24 hours turnaround) after being sighted. * user can use their own defined words immediately, however every day or so the moderator will do a "cleanup of the words" before it becomes available to everyone else. If the definition is very different between the user and moderator, then the moderator can push the user definition into "notes" and change the existing user list.
