Pvp heads drop on every pvp death with no cooldown between them.

Pvp heads don't drop at all (for normal players and moderators).

What dark just said before may be a admin thing, but I am uncertain of it. I do know that once werdnaz had killed theschlozmeister multiple times and got multiple heads each time with no cooldown. This was also when heads were working normally for other players.
