AQ+CRF broken

When AQ and CRF are enabled together, the output will be broken with a lot of artefacts. Though the PSNR and SSIM calculated will be, strangely, quite high.

It can be easily reproduced by using both --aq-mode 1 and --crf <int>.

It's reproducible since AQ and CRF were implemented. However the artefacts generated vary from versions.
