Make principles chapter optional in the main text

We have to decide whether to make Ch 2 (feedback principles) appear in the printed version of the text or not. If it does not appear in the printed version, a number of changes will be required:

Change the preface for the printed and electronic versions so that we include explanatory text about Ch 2 as being available only in the electronic version of the text.
Make sure that there are no references to Ch 2 in the primary text (should mostly be the case already and we should probably do this change irregardless of where Ch 2 ends up).
Renumber the chapters in a consistent way between printed and electronic versions. We can probably do this by renaming Ch 2 as "Supplement A" and Ch 15 as "Supplement B" and putting them in the appropriate locations.
