Fix Windows build, recent 2020-06-01 changes broke it.
Create the tests/testserver/etc/server.pem file on Windows.
On Windows skip http_persistent.test and ns_proxy.test for now.
Move ifdef to be only around nsconfig.h, not the whole pthread.c file.
Use tcltest constraints to skip problematic Windows tests.
