Mixed line-breaks in HTML output

When an HTML input has
r
n as linebreas and some <pre> elements, the output has mixed linebreaks:
n in <pre> and
r
n outside (on Windows).

This seem to be due to the skeleton writer where there is no linebreak conversion done at all. The skeleton needs to have a linebreak type specified and convert any text content to it.

Support for using the linebreak provided by StartDocument.getlineBreak() in the skeleton writer has been added. Seems to work now.
