Self-issued OP message definitions belong in Messages spec

I believe that Roland's right that parts of the self-issued OP definition should be moved from Standard to Messages when they describe messages that are independent of the binding.

Self-issued OpenID Provider is not defined in Message. It should be since it's used in several places in that document. It is sort of defined in Standard but not stated explicitly as a new term.
