Add phone_number_verified with the same characteristics as email_verified:

"OpenID Provider has taken an affirmative steps to insure that the phone identifier was under the control of the user at the time of the verification. "

We will add this to enable the text to be reviewed. We will continue discussing this.

We may need to add warning text saying that just because an e-mail address or phone number was verified at one time, that doesn't necessarily mean that they are still under control of the user.
