formatting issues in ID3

1. see @!predefined_values was presumably intended to become a hyperlink to somewhere
2. the _ in `electronic_record` has vanished (or rather, has been interpreted as a request to apply italic until the _ in predefined_values)

@Joseph Heenan The PR #88 “[IDA] Markdown fix” is a fix for the problem.
