Patient size and weight was missing from dx extractor and size was missing from rdsr. Fixes #165

I put this into develop/0.5.1 as I have done reasonably extensive revisions to dx.py that aren't yet in charts branch. There is nothing of course to stop you from making the same changes to the version you have. Or just copying the whole file. I think.

NEED to get the database stuff sorted. Arrghh!
