Fails to import Toshiba Cath Lab RDSRs

Initial error is due to the RDSR having patient orientation information that is incorrectly coded in the extraction routine. The incorrect database fields were fixed in the last release.

Changed Patient Orientation, Patient Orientation Modifier and Patient Table Relationship to use the newer cid fields. Added in some TODO information regarding where the Projection Eponymous Name field should be, both in RDSR and in models. Refs #55, #227

Patient Orientation Modifier should be at the same level as Patient Orientation. Corrected, and now seems to work. Need to check that the Siemens ones still work, and verify the data actually is in the database. Refs #227
