Review dx.py extraction, particularly mechanical

Whilst working on issue #242 and #243, it has become apparent that there are several extraction attempts in distance measurements and mechanical that are looking for tags that are not in the registry of DICOM data elements, and therefore are never going to be populated.

These fields probably do exist in the RDSR templates.

Some of the fields might have equivalent that could be used instead.
