Respond.js missing from js folder

If you are using IE8 or earlier, then a couple of extra files are dragged in, except one of them hasn't been added to the js folder and gets a 404. See https://groups.google.com/d/topic/openrem/0QxTAtYHeI8/discussion

Adding in missing respond.min.js for IE8 and below, as well as updating html5shiv and moving to the min version. Fixes #282
