Added a Toggle data table button below every chart. I have tested this for every OpenREM chart. References and fixes issue #422.

Updated export-csv.js JavaScript so that two decimal places are shown in the table for y axis data, and integers shown for frequency data. csv and xlsx exports will still contain all decimal places. References issue #422.

For fluoroscopy page: Automatically hide chart data table when full screen button is clicked, and also hide data table button. This prevents problems that may arise if a user clicks on the data table button when in full-screen chart view. I need to roll out these changes to CT, MG and DX pages. References issue #422.
