Remove 'Set to None' fix for missing attributes causing crash on export

Propose to remove these if statements now the modification made for patient age filtering have implemented a better solution - see #48 and #49

Model for general equipment module attributes didn't have a null=true flag. Can't see any reason why it would need to not be null, so this has been added. Will necessitate a South migration. Refs
