Add docs for conquest for Toshiba RDSR creation

Refs issue #480

Documentation currently doesn't cover how to configure conquest to accept the CT image objects retrieved by the --toshiba QR function and how to pass them to the new extractor function.

Also consider if the conquest examples could be improved, either based on those added to stuff/conquest or by linking to them in the repo on bitbucket as examples.
