Make skin dose calculation failure more explicit

I would like the skin dose failure message to be more explicit.

At a minimum, we know that only the Siemens frame of reference zero position is catered for, only copper filtration is considered, and all the couch/mattress considerations are modelled on one Siemens system.

It might be good to detect if the calculation has failed based on missing kV (ref #558) or missing patient orientation modifier (ref #557), but I think these are secondary to just being clear in the first place what will and won't work.

I'll create another ticket to cover not trying to calculate the skin map in the first place.
