Toshiba Fluoro RDSR with dGycm2 DAP units

As raised by Leo Romijn in this Google group message the Toshiba (Canon) Ultimax records the DAP in dGy.cm2 which is not catered for by the import routine.

This is a non-conformance to the standard, but we should try and fix it anyway.

The other issues in that conversation are fixed by issue ref #693
