Remove unused DICOM tags from study-query

During study-query a number of patient related information is queried while not used. Remove these tags from the query.

I had suggested 0.9.2, but we’ll need to run another beta to check the django-reverse-js install so I’ll throw this one in too: it shouldn’t make any difference to the C-FINDs…

Refs issue
#753Removed PatientAge and PatientBirthDate from the study-query. I didn't remove PatientID and PatientName although it seems it is not required by the standard, some PACS systems do require them.
