create new database section missing from phpliteadmin gui

What is the expected output? What do you see instead? See attached screen shot

What version of the product are you using? On what operating system? v1.9.1 running on Apache 2 Linux Mint 12

Please provide any additional information below.

this a new installation of phpliteadmin and I suspect that a PHP extension may be missing.

``` The problem was due to not having write permissions to the folder wherer the DB's will live. A quick chmod and all is well. Sorry for the panic....NR ```
