Names formatting, final tweak: do not attempt to group-sort citations in numeric styles.

Complete refactoring of names formatting code, and introduction of grouped citation sorting in author-date styles, and fixes for a couple of small issues concerning capitalization and punctuation, all as one, great, massive unwieldy checkin from a fork of the main code. The individual patch files for most of these changes will be checked in later, for reference in case HG doesn't carry the logs in the merge.

Monster checkin. Most of the volume here is made up of syntax changes from a jshint trawl through the codebase. Unfortunately, this also includes changes to implement grouped sorts within a citation, and probably some other tweaks as well. The main impact of the grouped sort changes can be found in node_citation.js, api_cite.js, registry.js, and util_names_output.js.
