Add a "Multiple Choice" test type

In our daily Ortho tests there is one test which requires a user to select an option from a drop-down. I suspect we could work around this in the short term but it may be a worthwhile enhancement.

Would require modifying the Test model to include a //choices// CSV field and also require some (client & server side) work on checking pass/fail status.
