Document CCTK_GFINDEX3D and friends in reference manual

I believe the reference manual does not describe any of the CCTK_GFINDEX* and CCTK_VECTGFINDEX* functions.

Both are mentioned in the UsersGuide - not the ReferenceGuide. Should we document them in both?

Everything needs to be documented in the reference manual. Missing GFINDEX3D, one of the most used flesh functions, is a major problem, and is a defect in the documentation.

in that case: None of the old reduction interface in section "C1.7.4" use the users guide http://einsteintoolkit.org/documentation/UsersGuide/UsersGuidech9.html#x13-110000C1.7 seems to be described in the reference manual anymore. Is this because they are not to be used anymore (some quite useful, eg. CCTK_ReduceLocScalar).

This is just one of many problems with the documentation, and they are not all going to be fixed by the release. This is not a new problem, and has been present for many releases. Therefore I am removing the release milestone for this release.
