CarpetReduce uses lsh for index calculations

CarpetReduce (e.g. reduce.cc:644) uses lsh to calculate GF indices. This is wrong in case lsh!=ash. Either use ash or the Cactus macros (which would probably be even better).

Fixed in git has of ba9d75ac "CarpetReduce: Support array padding (cctk_ash)" of Carpet (4 years, 9 months ago).
