Box Chart improvements

• Change title to show name of tool, not instructions
• Layout of the date selector below the workout list is confusing, and different from other tools.
• Reduce the form to one submit button
• Search filter on keyword
• Toggle all remains checked if you first check toggle all, but then deselect workouts. It should get unchecked if all workouts are not selected.
• Add annotations
