Clean up code-less "countries" in initial report

Initial countries included with a new list are: - Madeira - Kerguelen - Galapagos
etc...
These are just thoroughly confused, often: they might have matching states in all-states.csv, or have their species included in the country's checklist.
Do something that isn't broken. Ideally with Galapagos, for example: - Merge the un-IDd Galapagos with "EC-W" Galapagos state of Ecuador. - Include Galapagos as part of an Ecuador country checklist but not South America.

Madeira, Galapagos, Azores area all now un-duped; but they don't yet show up as part of one's Portugal or Ecuador list. That's still to-do.
Still need to move Kerguelen into French Southern Lands, and tie Macquarie Island to Tasmania.
