Can't create pull requests, they always result in a 500 error (BB-8048)

I can't make pull requests, i keep seeing a 500 error. I checked in Chrome Developer Tools and saw this on the 500 error page in the response, hopefully this helps identify the problem:

    Hi Kevin,

    I've deployed a fix that should prevent the 500 error. However, I was never able to fully reproduce the problem, so it's possible that you may still see some weirdness when creating pull requests. If so, please update this ticket with any relevant information.

Tip: Filter by directory path e.g. /media app.js to search for public/media/app.js.
